Instructions for completing your application:

1. Please type all information.
2. Use one form per person. If you are submitting a printed application, you may photocopy the original or call the office at (202) 628-2800 to receive another form. If you want to be scheduled with a friend, you must mail your applications stapled together.
3. Committee Choice: review the list and fill in your choice (s) on the registration form.
4. Committee assignments are made on a first come, first served basis, so please submit your application early. You will receive your assignment from the Festival office by mail. If you sign up by April 15, your name will appear in our Festival Guide magazine. Volunteers who want to work together should mail or deliver their forms stapled together.
5. Volunteers 14 years of age or under must be accompanied during their assignment by an adult authorized to supervise them.
6. Complete the form, check the agreement box, and click SUBMIT.

Training
All volunteers must attend one of the training sessions held during the weeks prior to the Festival. These sessions provide you with all the necessary details and logistics of the Festival. Training will take place at The Hilton Washington & Towers located at 1919 Connecticut Avenue, Washington DC 20009.

Please plan to attend ONE of the following training sessions:

Tuesday, May 18, 6:00pm-8:00pm
Thursday, May 20, 6:00pm-8:00pm

Schedule With a Friend:
If you would like to be scheduled with a friend(s), please list their name(s) below. Remember that they must also complete and submit their own volunteer applications separately. While we make every effort to schedule friends together as requested, committee positions may be previously filled, so we cannot guarantee requests will be fulfilled. If you are submitting print versions of applications, and you would like to be scheduled with a friend, please make sure to staple your applications together and submit them together.

I agree to attend one of the training sessions and to adhere to the policies and guidelines regarding volunteer work for the 2004 International VSA arts Festival. (An authorized adult must agree to supervise the volunteer if 14 years of age or younger.)
